eHealth Plan is a digital tool for managing chronic diseases and preventive care through personalized 12-month health plan, developed by i62 and family doctors.
Accessible via mobile devices, eHealth Plan helps patients track progress and communicate with doctors. Doctors monitor health, activity, and medication adherence through a dashboard. Plans include health goals, medical targets, and lifestyle changes.
Success: eHealth Plan improves medication adherence by letting doctors monitor prescriptions. In trials, 87% of high blood pressure patients met control targets in 12 months, compared to 44% with offline treatment.
